How to Apply
1. Online Application: Complete our online intake form to get started.
2. Print & Mail Application: Download and print this form, then mail it to: 1208 Fayetteville St., Durham, NC 27707
3. Refer Someone You Care About: Know a family who could use support? Submit our referral form to connect them with care.
4. Fax Referrals: If you’re unable to use the online form, fill out our printable referral form and fax it to 919-500-7616.
